No.... The state of California allows for direct patient access, which means a doctor's referral is not needed to be seen for physical therapy services.
If you are a cash pay patient, you can be seen by a physical therapist for 12 visits, or six weeks, whichever comes first, without a doctor's prescription. If you need continued treatment beyond that period then you would need to get a prescription from your doctor.
Completed registration forms from your registration packet. You will be mailed a registration packet which includes information about the clinic and your registration forms. Please fill out all of the forms before you come in as this will save you time.
Your photo ID.
Your insurance card, if any.
Your physician referral, if any. If your physician was planning on faxing the referral to our office, please call to confirm that we have received it before your first visit.
Comfortable clothing, including shoes and socks, that are appropriate for exercise. We have lockers and a changing room.
